LIFT JAX – Advancing Community Wellness with Actionable Solutions

LIFT JAX knew that health outcomes in their target neighborhoods were lagging, but understanding the root causes—and how to address them—required deep listening and strategy. They turned to Wirth Works to assess the true wellness needs of the community and translate them into action.

We conducted stakeholder interviews, analyzed environmental data, and surfaced the lived realities that often go unseen in traditional health planning. From there, we developed a pilot strategy to embed a “community health connector”—a trusted local advocate linking residents to care—backed by a compelling funding proposal.

The results were immediate and powerful. Funding was secured, partnerships were forged, and within months, the new program was launched. Today, uninsured residents are receiving routine screenings and health referrals, and thanks to a concurrently- launched City of Jacksonville telehealth program, the number of individuals in Out East without primary care access has dropped by 25%.
